3-Way Repeat-Back

When information is transferred…

  • Sender initiates communication using Receiver’s name. Sender provides a request or information to Receiver in a clear and concise format.
  • Receiver acknowledges receipt by a repeat-back of the request or information.
  • Sender acknowledges the accuracy of the repeat-back by saying, “That’s correct!” If not correct, the Sender repeats the communication.

3-Way Read-Back

The same as the Repeat-back, except…

  • The receiver writes down the information, request or order and reads back what they have written.

Example of a 3-way Repeat-back or Read-back:

  • Nurse: “I’m calling because Joan Smith’s blood pressure has dropped to 78 over 51.”
  • Physician: “Joan Smith, blood pressure, 78 over 51.”
  • Nurse: “That’s correct.”
